Code changes:  

* fix docs a bit.  

* uninline lp_pps_echo(): we take its address.  

* remove unneeded and undesirable casts of void*'s.  

* coding-style fixes.  

* various changes to the use of the timer API.  

* Remove lots of inlinings. The compiler gets this right.  

* DEFINE_SPINLOCK is required: SPIN_LOCK_UNLOCKED defeats lockdep.
