In short: I don't want to do it in 2009 or later.

I did the RTAI-approach years ago, (2.2.x kernel) when SMP, rt ...  was not in sight.
In These good old  times a simple switch of console caused a BKL-delay of 200ms.
So you were forced to use RTAI.

Things have changed a lot!

This time has other challanges (SMI, APIC, loss of "direct lines" /dev/ttyS* /dev/lp*).
