I have applied the patch from bugzilla to ntp-dev-4.2.5p236-RC and it does 
not work for me.  Ntpd does not sync for me even after several hours of 
waiting.  It does seem that refclock_nmea now "sees" the pps pulse, it is 
just that ntpd won't sync to either GPS_NMEA or PPS.  I haven't had a 
chance to investigate further, so it may be ntpd is not configured 
properly.
